Where We Serve

Providence Senior and Community Services offers a full range of services for people of all ages – especially those facing chronic or life-limiting illnesses – integrating hospice, home care, assisted living, nursing homes, skilled nursing, infusion, care for the elderly and housing for the poor and vulnerable.

With 32 locations across three states and a staff of more than 2,700, our ministries touch the lives of just over 18,390 people every day. At the end of 2013, on a daily basis, we were serving:

2,509 patients through our hospice and palliative care programs.
1,433 people through home telehealth (Lifeline) services.
1,729 patients through home health services.
783 residents through our assisted living and skilled nursing facilities.
472 people through our Program of All-Inclusive Care for the Elderly (PACE).
793 residents living in our supportive housing locations.
And thousands more in programs like bereavement services and home infusion care.
