To access the required documents and forms for hospital clearance, please visit the Providence Prep Portal.
If you have already been assigned a clinical rotation at Providence Regional Medical Center Everett, please check in with the Medical Staff Office at 425-261-3929 in the Medical Staff Office at least one week prior to beginning your rotation for hospital clearance.
If your institution is not listed, please have your Graduate Medical Education Coordinator contact our office to begin the contracting process. Please note that the school and student must contact the proctor directly and set up their rotation.
