Billing, co-payments
If your health plan requires a co-payment for office visits, please plan to pay the receptionist before you see your doctor or other provider. If you are not covered by an insurance plan, we ask for a $75 deposit at the time of your visit. You will be billed for the remaining balance.
Billing for treatment related to a motor vehicle accident
If you are being evaluated and treated for injuries caused by a motor vehicle accident, it is your responsibility to inform the receptionist and physician upon arrival. If you carry a personal injury protection (PIP) plan on your policy, we will bill them as a courtesy to you. You will also be responsible for payment of services relating to the accident. We will provide you with documentation to submit to your auto insurance company or attorney for reimbursement.
Pay via personal check, cash, Visa or MasterCard
We accept payment by personal check, cash, Visa or MasterCard. If you need help with your bill, call our Physician Billing Office at 425-317-0699. Our billing specialists will help you evaluate your payment option plan information.
